Learning outcome: 2. Understand how working in partnership with others contributes to children’s well-being during transition to school
Assessment criteria: 2.2 Describe the information required to enable the school to meet the individual needs of the child during transition

• To meet the individual needs of the child during transition, the school will need a copy of their EYFS profile
• This will state the child’s level of development in each of the areas of learning (Early Learning Goals) as either emerging, expected or exceeding
• Other information that will be useful to the school can include:
  • The child’s friends
  • The child’s likes, dislikes and interests
  • Information about any Special Educational Needs and Disabilities that the child has
  • Other assessments that have been performed
