- Finalise Goals and Objectives: Agree on specific, measurable, achievable, relevant, and time-bound (SMART) goals and objectives that reflect the individual’s aspirations and the collective insights from the review process.
- Outline Support Strategies: Clearly define the support strategies to be employed, including any assistive technologies, physical aids, or personal support arrangements.
- Assign Roles and Responsibilities: Specify the roles and responsibilities of all parties involved in supporting the individual, ensuring clear understanding and accountability.
- Establish Communication Protocols: Agree on how and when communication will take place among the individual, carers, family members, and any professionals involved, to ensure ongoing collaboration and support.
- Plan for Monitoring and Review: Set dates for regular monitoring of the plan’s implementation and future review meetings to assess progress and make further adjustments as necessary.
- Ensure Accessibility of the Plan: Make sure the revised plan is accessible to all relevant parties, in formats that meet the individual’s needs and preferences.
- Confirm Agreement: Obtain confirmation and agreement from the individual and all stakeholders on the finalised revised plan, ensuring everyone is committed to moving forward together.
- Document and Distribute the Plan: Document the agreed-upon revised plan in detail and distribute copies to all involved parties, ensuring everyone has the latest version for reference.
