Unit: Facilitate support planning to ensure positive outcomes for individuals and to support well being
Learning outcome: Be able to facilitate the implementation of support plans in partnership with the individual and others
Assessment criteria: 4.1 Agree how a support plan will be carried out with an individual and others

• After creating a support plan in collaboration with an individual and others, it should be discussed how it will be carried out
• Others can include the individual’s family, their advocate, team members and other professionals (e.g. communicty nurse, dietician etc.)
