Unit: Facilitate support planning to ensure positive outcomes for individuals and to support well being
Learning outcome: Be able to develop a support plan to meet the identified needs of an individual
Assessment criteria: 2.5 Work in partnership with an individual and others to identify options, resources and preferences in relation to an assessment

• Options, resources and preferences relating to assessment should be discussed in partnership with the service user and others
• Others can include their family, advocate, occupational therapist, social worker etc.
• E.g. an individual that has a hearing impairment could discuss options with an occupational therapist for ensuring that they are alerted when the smoke alarm goes off
