• A strengths-based assessment process will focus on the intended outcomes (e.g. for an individual with dementia to maintain their current social activities and network)
• This should be agreed in collaboration individual to maintain a person-centred approach
• Others may include the individual’s family and friends, their advocate and other professionals, such as their psychologist, doctor, social worker etc.
