Assessment criteria: 1.1 Illustrate how a range of people, organisations and community groups can contribute to achieving positive outcomes for those using a service
- The successful achievement of positive outcomes for individuals using care and support services is reliant on the contributions of a range of people, organisations and community groups
- For example, in the case of a young person with a learning disability may have the goal of moving out of their parent’s house and living on their own:
- Their parents may contact the local authority to discover the services available
- The local authority will assess the individuals’ needs and commission appropriate care and support services
- Care and support staff will support the individual to gain the skills they need for independence
- A local community cooking group may support the individual to learn cooking skills for a range of meals
- Positive outcomes mean that all parties must work effectively together – this is known as partnership working or integrated working
