• In 2008, the Department of Health published a national framework for end-of-life care
• The end-of-life care pathway consists of six steps:
  1. Discussions as the end of life approaches
  2. Assessment, care planning and review
  3. Coordination of care
  4. Delivery of high-quality services in different settings
  5. Care in the last days of life
  6. Care after death
• Local authorities use this framework to develop their local end-of-life care pathway – you will need to be aware of how the pathway is implemented in your region
