• Tools that may support an individual and others in end-of-life care can include:
  • Abbey Pain Scale – used for the assessment of pain of individuals that are unable to communicate/articulate
  • Malnutrition Universal Screening Tool (MUST) – screening tool for individuals that are malnourished, obese or at risk of malnourishment
  • Barthel Index – used to measure performance in daily living activities and mobility.
  • Waterlow Score – calculates an estimated risk of an individual developing a pressure sore
