- All individuals receiving care and support services will have their own personalised care plan
- Care plans describe the needs, wishes and preferences of the individual and how care and support activities should be carried
- The format of care plans varies between organisations. This can include whether they are digital or paper-based. However, they should all contain similar information.
- Some of the information contained in a care plan includes:
- Any medical conditions the individual has
- How best to communicate with the individual
- The individual’s meal preferences
- The strengths of the individual and their level of independence
- How care tasks should be carried out
- The things that are important to the individual
- The individual’s family network and next of kin contact details
