• Support plan reviews should take into account feedback from all stakeholders, including:
  • the individual receiving care
  • their family/friends
  • their advocate
  • care staff
  • Other healthcare professionals
• Assessed risks should be reviewed to identify if the risk assessment is still necessary and if any changes are needed
