Unit: Facilitate support planning to ensure positive outcomes for individuals and to support well being
Learning outcome: Be able to facilitate the implementation of support plans in partnership with the individual and others

• As a lead practitioner, it will be your responsibility to ensure that the support plan is implemented correctly
• This will mean regular reviews with all stakeholders to ensure it is working
• It may mean reviewing and changing some areas of the support plan over time
• Any areas that are not being implemented correctly will need to be addressed
