• During assessment, you will collaborate with the individual and others about how care and support services will be provided and this will be recorded in their support plan
• Support plans should also be reviewed regularly in collaboration with the individual
• You can assist an individual to make informed choices about their support by providing them with factual information
• You may help them to weigh up the pros and cons of each option
• You should not allow your personal views to influence their decisions
