2.1 Support an individual to make choices over decisions to meet their identified needs, preferences and wishes

Qualification: Level 4 Diploma in Adult Care
Unit: Facilitate support planning to ensure positive outcomes for individuals and to support well being
Learning outcome: Be able to develop a support plan to meet the identified needs of an individual
Assessment criteria: 2.1 Support an individual to make choices over decisions to meet their identified needs, preferences and wishes

  • Work collaboratively with an individual to indentify their needs, wishes and preferences
  • Provide individuals with the information they need to make informed decisions about their lives
  • Be factual – do not let your own personal views influence an individual’s decisions
  • E.g. an individual with agoraphobia (fear of open spaces) wants to gain a qualification. Going to a local college could help them to overcome their phobia, however it may also increase their anxiety so much that they are unable to study for their course. Discussion with the individual about the pros and cons of each will help to determine what is the correct choice for them.
