- Outcome-based practice involves focusing on the end-results or impact that care and support services have on an individual’s life
- This is person-centred because it supports service users to achieve the goals that are important to them
- E.g. an outcome for an individual with a learning disability may be that they want to be able to cook for themselves rather than being reliant on others
- Approaches to outcome-based practice include:
- Results-based accountability – success is measured by reviewing services against the outcomes agreed with service users
- The Logic Model – a framework for describing how services will achieve outcomes. Usually a graphical representation that shows the inputs, outputs, outcomes, context and the relationships between them all
- Outcomes management – a system for identifying outcomes, planning activities to achieve them, tracking progress and evaluating results
- Outcomes into practice – how outcomes can be used in day-to-day practice
