- You should reflect on how your own records and reports have been used to provide the basis of judgments and decisions
- E.g. perhaps you wrote a report for senior management about how your team are not getting the correct work-life balance, which resultedin the budgeting of additional staff
- E.g. A pattern emerged in a service user’s records that identified their well-being suffering when they had too many late nights in a row. After discussions with the service user, it is agreed that staff are to remind the service user to go to bed before 10pm five out of seven nights a week.
