  • Evidence for quality can include client feedback surveys, complaints logs and accident books
  • These records can be used to identify areas where an organisation may not have reached acceptable standards of care
  • Once identified, they can be investigated to gather more information and try to identify the root cause
  • The root cause can then be addressed to prevent similar issues in the future
  • In addition, themes or patterns may be found in their records (e.g. if a particular error is common in the late evening, it could indicate that staff are tired and the timing of the task could be changed to earlier in the day)
