• All systems, processes and approaches (including those related to person-centred and outcomes-based practices) should be regularly reviewed because:
  • It embodies a culture of learning and continuous improvement
  • It assesses their strengths and weaknesses and addresses any areas for improvement
  • It forces us to think about their efficacy, rather than assuming everything is fine
  • It allows us to gather feedback about what others think could be improved – this can include looking at existing records, such as complaints and comments
• This should form part of a robust quality assurance system
