- Outcomes-based practice focuses on the needs and aspirations of the individual receiving care and support services rather than what a practitioner thinks the best outcome should be
- It is based on the principle that individuals are experts in their own lives and are the best person to know what the desired outcome of their care and support should be
- For example, if a frail and elderly individual is admitted to the hospital because of a fall, a practitioner may think that when they are discharged they should move into a care home. However, the individual themselves may want to stay at their home to keep their independence and ties with their community. An outcomes-based approach would take the individual’s wishes into account and look for ways to adapt their home so that they can remain safe.
- Person-centred practice is an approach that puts an individual receiving care and support services at the heart of their care planning delivery
- Individuals (and their families) should be treated as equal partners in their care planning and delivery, be involved in the process and make their own decisions
- Person-centred practice is based on person-centred values, which include respect, dignity, choice, privacy, partnership, independence and control.
