Assessment criteria: 2.5 Explain how systems and processes can be designed to engage with individuals and those important to them in day-to-day practice, decision-making and review
- There should be systems and processes in place to ensure that individuals and those important to them (e.g. family, friends, carers etc.) can be involved in day-to-day practice, decision-making and review
- Systems and processes could include:
- Asking an individual who they would like to participate in the care planning
- Ensuring there are records of the people that are closest to the individual
- Ensuring these people are contacted when and encouraged to participate when there is a review due
- Nurture a culture of valuing and respecting everybody’s contributions
- Arranging for an advocate to speak on behalf of the individual if they are unable to do so for themselves
