• Care Plans are an essential tool for ensuring that person-centred values are applied in a care setting
• Care plans should be written in collaboration with the service user, with input from other stakeholders, such as their family and other professionals
• Care plans will detail each individual’s unique needs, wishes and preferences (e.g. how they prefer to communicate, what they like to eat etc.)
• Each carer will have the information they need to provide consistent care in a person-centred way
