• It is important that information about an individual’s needs, wishes and preferences for their end-of-life care is passed on because:
  • It ensures that the individual’s wishes are taken into account should others be required to make a decision in their best interests
  • It ensures care and support services are tailored to the individual’s wishes
  • It safeguards the interests of family and carers
  • It gives individuals and their loved ones peace of mind that the future is taken care of (which can reduce stress and worry and improve quality of life)
• It should be remembered that information should only be shared with the individual’s consent
