  • Serious Case Reviews (SCRs) are performed by Local Safeguarding Children’s Boards (LSCBs) when a child dies or is seriously harmed as a result of abuse or neglect
  • Reports to the board are made by agencies and individuals that had a responsibility for the child
  • The primary goal of an SCR is to identify lessons that can be learned and how agencies can reduce the likelihood of similar incidents in the future
  • SCRs can also be used to improve the way that agencies work together in partnership
