  • As well as preparing a child for a transition, an Early Years Practioner should provide support for a child during a transition or significant life event
  • Again, partnership working with the child’s family and other agencies, organisations and professionals is essential so that information about the child can be shared and all partners work towards the same common goals and outcomes
  • In some cases, more specialised support may be required for the child and their family and so the Early Year’s Practitioner may make a referral or signpost the family to support that is available
  • Early Year’s Practitioners should encourage children to express their emotions and discuss their feelings
  • The child’s Key Worker should be on hand to respond to any concerns or questions that the child has and toprovide emotional support – this may also be facilitated through play activities
  • The Key Worker should also make observations of the child and keep a record according to organisational policy/procedure
